Listed below are a number of forms related to the Utility Billing department.
Water Service Application
All new Residential Water/Sewer service requests will be billed $100.00 which includes an application fee of $25 and a refundable deposit of $75. Commercial accounts will be billed $300, which is a $25 application fee and $275 deposit that is refundable. All accounts require a deposit and application fee. All information must be completed with a copy of the account holder(s) driver's license and rental agreement if leasing. Applications are processed on normal business days, Monday - Thursday from 8:00am - 4:00pm. All application received after 4:00pm will be processed the next business day.
New Meter Application Online form to set a meter at a new residential/commercial property location. This form is to be used for builders/contractors when establishing meter connection. All requests will be billed $100.00 which includes a connection fee of $25 and a refundable deposit of $75. Commercial accounts will be billed $300, which is a $25 connection fee and $275 deposit that is refundable. All accounts require a deposit and a connection fee. All information must be completed with a copy of the applicant's driver's license and a copy of permit. If the meter is not ready to be set on the date indicated on the application the request will be voided until further contact and an additional $25.00 will be charged for each connection attempt. Monthly bill will include water and sewer charges.
Water Disconnection Request
Disconnection requests received by 4:00pm can be processed the same day. Refund checks (if applicable) will take 4-6 weeks for processing.
Transfer Water Service within City of Forney's city limits
Online form to move service from one City of Forney address to a new address within the City of Forney's water district. Water may overlap no more than 7 days at both addresses. Please use the New Water/Sewer Service Request above if the overlap of service is needed greater than 7 days. All information must be completed with a copy of the account holder(s) driver's license.
Water Leak Adjustment Request Form
Online form for the account holder to request a credit to the account offsetting the cost of lost essential water. Irrigation, outdoor faucets, and pool repairs do not qualify for a leak adjustment. Submit after the leak has been repaired. Submission is not a guarantee of approval.
Change Request Form
Online form for the account holder to update mailing address and/or add a secondary account holder.
Bank Draft Application
This form will authorize the City to draft your account balance on the due date of each month from your checking/savings account. There are no fees associated with automatic bank drafts.
Payment Arrangement Request Form
Online form for account holders with a minimum of 3 months credit history on the account requesting an extension to prevent water service disconnection. Arrangements are only granted once every 3 months and a $10.00 fee will be included in the arrangement amount to be paid. Request must be received and accepted by 5:30pm one (1) business day prior to the scheduled cut-off date. The arrangement date is not to exceed 17 days past the due date.
